Birth & Death Certificates

Certified copies of Missouri birth and death certificates are provided by the Wright County Health Department Vital Records department, Hartville location, Tuesday-
A Notary Public is available to notarize affidavit for corrections, paternity affidavit, and out of state birth and death certificates.

Birth Certificates
Certified copies of birth certificates for persons born in Missouri from 1920 to present can be obtained in our office for $15.00 per copy. Cash, check, money order or credit card (processing fees apply) are all accepted forms of payment.
Request for Birth Certificate Application in Person
Birth certificates will only be issued to the following people:
NOTE: A photo I.D. of yourself will be required to receive a birth certificate.
Death Certificates
Certified copies of death certificates are available for deaths occurring from 1980 to present. Death Certificates are $13.00 for the first copy and each additional copy is $10 each issued to the same person in the same visit. Cash, check, money order or credit card (processing fees apply) are all accepted forms of payment.

Death certificates will only be issued to the following people:

Request for Birth or Death Certificate by Mail
Applications requested by mail MUST BE NOTORIZED before the birth/death certificate can be issued.
Applications not notarized will be returned. Please do not send cash in the mail.

Certificates for births, deaths, marriages and divorces that took place outside of the state of Missouri can be obtained by visiting the National Center for Health Statistics website.
